Today we would like to share a case involving a middle-aged female patient who experienced both sagging of the upper eyelids and puffiness in the lower eyelid area. This patient felt that her eyes had become increasingly heavy and tired-looking during daily activities.
Friends and colleagues often mentioned that she appeared fatigued even after sufficient rest. Over time, she became more aware of the changes around her eyes. She noticed that the skin above her eyes was drooping and her double eyelid line appeared loose and undefined. Additionally, the lower eyelids appeared puffy and cast shadows under her eyes, which contributed to a tired and aged look.
Such symptoms are common as the skin loses elasticity with age and the fat compartments in the lower eyelid shift forward. This patient had searched for information about Korean upper eyelid surgery and visited our clinic for a detailed consultation.
Upon examination, it was clear that she was using her forehead muscles to lift her eyelids unconsciously due to the weight of the drooping upper eyelid skin. The lower eyelids were also protruding due to fat herniation which intensified the impression of swelling and tiredness.
After thorough evaluation, it was determined that her concerns could be addressed through upper blepharoplasty and fat repositioning of the lower eyelids. Korean upper eyelid surgery involves more than simply excising excess skin.

As with any periocular procedure, there was some swelling and bruising immediately after surgery. However, these subsided with time and the natural contours of the eyes became more visible.
After recovery, the patient shared that she no longer felt the same heaviness in her eyes and noticed that they opened more easily without relying on her forehead muscles. She also received positive feedback from people around her who commented that she looked more refreshed and relaxed.
The shadows under her eyes had softened, contributing to a more youthful and calm appearance. This case illustrates how Korean upper eyelid surgery, when combined with lower eyelid fat repositioning, can improve the harmony of the upper and lower eye areas in a balanced way.
Patients often focus only on the upper eyelids or lower eyelids in isolation. However, in many middle-aged patients, a combined approach yields more natural results. The upper and lower eyelids age differently but are anatomically and visually interconnected. Therefore, surgery that considers this relationship tends to result in a more pleasing and functional outcome. Korean upper eyelid surgery can improve visual fields and reduce habitual forehead tension. However, when the puffiness of the lower eyelids is left untreated, the overall facial expression may still appear tired or unbalanced.
